Petition for Special Relief - this paperwork is solely for the purpose of requesting a temporary order of court to address an emergent situation. This petition does not provide immediate relief; if you have critical safety concerns, please contact local authorities. The completed document is to be filed in the Prothonotary's Office with a payment of $25.00.
Petition for Contempt - this paperwork is solely for when an active custody order is not being followed. The completed document is to be filed in the Prothonotary's Office with a payment of $25.00.
Agreement - this paperwork is solely for when all parties can agree on a custody schedule and no court intervention is needed. All agreements will need to be notarized and signed by all parties. The completed document is to be filed with the Blair County Custody Division; if this is an initial filing, there will be a $149.50 filing fee.
Petition to Intervene - this paperwork is solely for when a person wants to be included on an existing custody case. The completed document is to filed in the Prothonotary's Office with a payment of $25.00.
Continuance Request - this paperwork is solely for the purpose of requesting that a hearing or conference be moved to a different date. All parties must be notified of this action. The completed document is to be filed in the Prothonotary's Office; no payment necessary.
Agreement to Withdraw - this paperwork is solely for the purpose of canceling a pending action or closing a case. The completed document is to be filed in the Prothonotary's Office; if you are canceling an action, there is no fee, but if you are requesting to close a case, there is an $11.00 fee.
Request for Evidentiary Hearing - this paperwork is solely for when an agreement could not be reached during mediation, and one or both of the parties would like to have pre-trial conference scheduled. The completed document is to be filed with the Blair County Custody Division; no payment necessary.
Notice of Proposed Relocation - this paperwork is solely for the purpose of requesting court approval to move when such move would significantly impact the other party's custodial rights. The completed document is to be filed in the Prothonotary's Office. No payment necessary.
Change of Address Form - this paperwork is solely for the purpose of updating one's contact information. The completed document is to be filed with the Blair County Custody Division. The form can be submitted via email or by in-person delivery. No payment necessary.
In Forma Pauperis (IFP) - this paperwork is solely for the purpose of requesting a fee waiver when your financial situation has rendered you unable to pay the fees associated with your custody action. If you are filing more than one petition, you must file a separate IFP for each filing. The completed document is to be filed in the Prothonotary's Office with the petition(s) you are requesting the waiver for.
